Effective October 1, 2020, Apple-a-Day Pediatrics will begin to charge patients when they do not arrive for scheduled appointments. Failure to cancel or reschedule the appointment 24 hours in advance will result in a $40 “No Show” fee. This fee will not be submitted to your health insurance; it will be charged directly to the guarantor. We understand that flexibility is important, therefore patients are allowed one “free” missed appointment charge. In the event of 3 or more missed appointments, we may ask you to seek services from another practice and/or be assessed a $50 fee. If you are 10 minutes or later for your appointment, you may be asked to reschedule.
Miscellaneous Fees
Returned Check: $40 fee and all future payments must be cash or credit card
No Show: $40 fee
Medical Records: $25 fee for the first child, $10 for any additional children
Work form for parents: $25 fee
Disability Forms: $15 fee
Daycare Forms: $15 fee (if not requested at well check appointment)
Camp, Sports, or School Forms: $15 fee (if not requested at well check appointment)
Medication Administration and Food/Allergy/Asthma plans: $15 fee (if not requested at appointment)
Forms that require ONLY a physician signature: no charge
